For healthcare and social care professionals. We are building a referral pathway that makes it as straightforward as possible for professionals to refer patients and clients into our care. While completing our CQC registration, we are already accepting referral enquiries.

Who can refer

• GP practices and primary care networks

• Hospital discharge and Discharge to Assess teams

• Local authority social care teams

• Community health teams and district nurses

• Mental health community teams

• Independent social workers and care managers

Response commitment

We respond to every referral enquiry within two hours during office hours (Monday to Friday, 9am–5pm).

Note

We are currently completing our CQC registration. We can complete care assessments and prepare ahead of registration being confirmed.
